Quebec St, and any other locations for training, clinics, games, and any other basketball related activities including strength and conditioning (the "facilities") owned/leased and operated by Basketball Tao Dojo, Basketball & Beyond, Brian Howard, who collectively together with their owners, partners, agents, employees, agents, independent contractors, consultants, are referred to herein as "Basketball & Beyond". For and in consideration of access to and use of the equipment, events or activities of Basketball & Beyond, I hereby relinquish, discharge, and release Basketball & Beyond now and forever, from any and all claims, demands, liability or causes of actions for personal injury, property damage, sickness, or death occurring to me or to anyone else caused by or arising from my presence at or use of Basketball & Beyond facilities, equipment, events or activities, including but not limited to injury, sickness, or damage caused by negligence of Basketball & Beyond or the owners, operators of the facilities, now and forever, from any and all claims or causes of actions by whomever or wherever made or presented caused by or arising from my presence at or use of the facilities, equipment, events, programs or activities of Basketball & Beyond.

It is my understanding that there are risks of minor, serious or permanent injury, sickness, or death in any recreational or sport activity and I voluntarily assume such risks. I acknowledge and understand that included in the scope of this waiver and release is any cause of action arising from the performance or failure to perform construction, maintenance, inspection, supervision or control of the facilities, equipment or premises or the cleaning thereof.

In the event there is an emergency and I cannot be reached, I hereby consent to whatever medical treatment may be considered necessary by the attending physician for me. In addition, I agree that in the event that I am injured or become sick as a result of participation at Basketball & Beyond facilities or events or activities, whether or not caused by the negligence of Basketball & Beyond, recourse for the payment of any resulting hospital, medical, or related costs and expenses will first be had against any hospital or medical insurance or any available benefit of mine.
